Prior to your first appointment, you will have an opportunity to engage with our intake
coordinator and complete all of your intake paperwork. This will provide you with an
opportunity to express in your own words, what you hope to accomplish in treatment. You will
also be supported in activating your patient portal.
Your provider will review your intake paperwork prior to meeting with you. When you meet with
your provider for the first time, it is their goal to get to know you better by compassionately
listening to your needs, while also conducting a comprehensive assessment with attention to
