How to Get Started

Getting started with CES typically involves the following steps:

  1. Submit a Request or Referral Form
    Complete the form below that best fits your needs.
  2. Administrative Review
    Our administrative team reviews your information to determine provider availability and next steps.
  3. Follow-Up & Scheduling
    We will contact you to discuss services, availability, and scheduling options.

Preparing for Your First Appointment

Once your appointment is scheduled, we will provide additional details specific to your provider and services. In general, you can expect:

  • Intake paperwork to be completed digitally* prior to your first visit
  • Information about telehealth access (if applicable)
  • Details about payment, insurance, and cancellation policies
  • Waiting room has snacks, water, and coffee available for in-person appointments
  • Digital waiting room for telehealth appointments (please join 5-10 minutes before your scheduled appointment)
